Muscat- Approximately 5-10% women are affected by Polycystic Ovarian Syndrome (PCOS) - a common endocrine gland disorder in women of reproductive age. Insulin resistance is a key feature in PCOS, as insulin helps regulate ovarian function. High levels of insulin as a result of insulin resistance can contribute to lack of ovulation, high androgen levels, infertility and early pregnancy loss. Diagnosis of PCOS should be considered when women present symptoms such as menstrual cycle dysfunction, infertility and hirsutism.

"A PCOS diagnosis must include a detailed history and a physical examination measuring blood pressure, weight and height. A routine pelvic and general physical examination should also be conducted, noting the presence of secondary sex characteristics, along with palpation of the thyroid gland for masses or enlargement. The ovaries and adnexae should be palpated for signs of enlargement and the presence of any tumours or cysts, both of which suggest PCOS. Presence of hirsutism, evidence of ovarian dysfunction and obesity should also be addressed. An ultrasonography can be especially helpful in establishing the diagnosis of PCOS" says Dr. P. Rama Devi, Senior Infertility Consultant and Managing Director, Al Hakeem Fertility Mother and Child care Centre.

The risk of PCOS can be reduced by avoiding obesity and following a non-diabetogenic diet, which reduces the risk of type 2 diabetes by increasing insulin sensitivity.

"The first PCOS treatment for infertility involves the administration of medication like Clomiphene Citrate, Metformin and hCG to stimulate ovulation. Medical procedures like IUI and IVF may also be suggested. It must be noted however that the success rates of PCOS treatment options to induce ovulation depends on the age of the women, the type of ovarian stimulation medication and other factors that contribute to infertility in the women. For these reasons it is advisable to consult a fertility expert for guidance" concludes Dr. P. Rama Devi.
