Dubai: Medcare, a pioneering healthcare provider in the region, recently treated a patient for respiratory difficulties through a microlaryngeal surgery using a laser machine (Right Medial Arytenoidectomy + Right Posterior Cordectomy). The patient presented with complaints of respiratory distress, noisy breathing and a high-pitched wheezing sound which originally started three years ago but had become progressively worse.

He had been involved in a car accident three years prior for which he was admitted to an ICU in a coma and artificially ventilated through a tube, via intubation, for three weeks. After the accident, the patient was unable to participate in any exercise due to fatigue and a suffocating feeling due to a lack of air.

Dr Muhammad Jabbr, Specialist ENT, Head and Neck Surgeon at Medcare Hospital Dubaiinspected the patient using a flexible laryngoscope to find bilateral vocal cord nerve paralysis making the vocal cords in paramedian position and caused the respiratory “chink” very narrow. It was suspected the patient attained this injury when ventilated through orotracheal tube in ICU. CT Scan of neck and chest was clear, which has been done to exclude any tracheal stenosis due to prolonged intubation.

Dr Jabbr performed a very delicate micro-laryngeal surgery using a laser (Right Medial Arytenoidectomy + Right Posterior Cordectomy) to widen the patient’s respiratory chink in the larynx, while preserving the voice quality. The operation needed incredibly delicate skill – if not performed correctly, there was a risk of permanent effects on patient ability to speak and risk of liquids aspiration to airways in addition to intraoperative respiratory obstruction risk which might needs more invasive surgery to make a hole in the neck to breathe. The operation went successfully and the patient was discharged the next day. If the operation had not worked in widening the patients respiratory tubes, he would have had to undergo a much more invasive tracheostomy surgery where a tube is inserted in the neck to breathe.  

“The patient presented with severe symptoms that did not let him breathe freely, and were impacting his ability to carry out activities or exercise. Our team at Medcare Hospital successfully carried out the delicate operation using the latest laser technology to open up his airways – he made an excellent recovery and now has very good voice quality and can exercise freely” said Dr Jabbr. “His condition was caused due to prolonged intubation while in the ICU after a car accident. It is important that patients are not artificially ventilated in this fashion for a prolonged period of time as it can have permanent damaging effects on their airways”.
